Registration
We will gather your information to set you up as a Continuum Health member. This will include demographic information, as well as some medical history.
Care Plan Selection
You will then be able to select the Continuum Care plan that you are interested in seeking improvement in. Each care plan will take you through additional selections, such as obtaining convenient laboratory testing (blood or semen analysis) prior to your visit. Continuum offers discounted laboratory testing to prospective members at local draw centers.
Pricing Quote
Receive an estimate of how much services may cost, prior to your appointment. It is possible to just schedule a consultation only as well.
Schedule Appointment
When you are ready to move forward, a small deposit will be required to secure your appointment. An appointment calendar will be sent to you at this time for easy convenient scheduling. Telemedicine options are available for patients in California (other states coming soon).
